The STOP-BANG questionnaire was developed and validated at the University of Toronto and is now used globally as the standard pre-screening tool for Obstructive Sleep Apnea before anaesthesia, surgical procedures, and clinical sleep evaluation. Each letter represents one risk factor.
Score 0–2 | Low Risk
Low probability of OSA. Formal sleep study not routinely indicated unless significant daytime symptoms are present.
Score 3–4 | Intermediate Risk
OSA is possible. Clinical evaluation recommended. Consider home sleep test or polysomnography alongside ENT airway assessment.
Score 5–8 | High Risk
High probability of moderate-to-severe OSA. Formal sleep study and urgent ENT evaluation strongly recommended.
Score ≥3 + Male | High Risk
A score of ≥3 combined with male gender significantly increases sensitivity for clinically significant OSA.
What Happens After a High STOP-BANG Score?
A high STOP-BANG score is the beginning of the evaluation — not the diagnosis. The next step is a formal polysomnography (PSG) sleep study, which measures your AHI (Apnea-Hypopnea Index), RDI (Respiratory Disturbance Index), oxygen nadir, arousal index and sleep architecture. This is interpreted alongside a nasal endoscopy to identify the anatomical site of obstruction — nasal, palatal, tonsillar or base of tongue — and then matched to the most appropriate treatment.
